Lupus eritematoso cutáneo: Una actualización narrativa

El lupus eritematoso cutáneo (LEC) es una enfermedad autoinmune crónica con manifestaciones clínicas heterogéneas, mediada por mecanismos inmunoinflamatorios complejos, especialmente la activación del eje del interferón tipo I. Un subgrupo de pacientes desarrolla enfermedad refractaria, asociada con mayor carga sintomática y deterioro significativo de la calidad de vida. Se realizó una revisión narrativa de la literatura en bases de datos biomédicas relevantes, incluyendo estudios originales, ensayos clínicos, revisiones sistemáticas y consensos clínicos. La evidencia demuestra que, además de las terapias convencionales como fotoprotección, antipalúdicos e inmunosupresores, han emergido terapias dirigidas con mecanismos específicos, como inhibidores de Janus cinasa, bloqueadores del interferón tipo I y anticuerpos anti-BLyS, que han mostrado eficacia en LEC refractario. Estos avances representan un cambio hacia estrategias terapéuticas más personalizadas, con potencial para mejorar el control clínico y los desenlaces en pacientes con enfermedad resistente.
