Percepción de enfermedad y síntomas de ansiedad y depresión en pacientes hospitalizados por COVID-19 en una clínica privada

Resumen

Objetivos: Determinar la asociación entre la percepción de enfermedad y los síntomas clínicamente relevantes de ansiedad y depresión en pacientes hospitalizados por COVID-19 en una clínica privada. Materiales y métodos: Estudio observacional, analítico y transversal realizado entre septiembre de 2020 y abril de 2021. La percepción de enfermedad se evaluó mediante el Brief Illness Perception Questionnaire (BIPQ) y los síntomas de ansiedad y depresión mediante la Hospital Anxiety and Depression Scale (HADS). Se utilizaron puntos de corte de 0-7, 8-10 y 11-21 para ausencia, síntomas leves/moderados y síntomas clínicamente relevantes, respectivamente. Se estimaron razones de prevalencia crudas (RP) y ajustadas (RPa) e intervalos de confianza (IC) al 95% mediante regresión de Poisson con varianza robusta. Resultados: Se incluyeron 226 pacientes hospitalizados con diagnóstico confirmado de COVID-19. El 46,0% de los pacientes presentó síntomas clínicamente relevantes de ansiedad y/o depresión. En las subescalas HADS, el 27,0% presentó ansiedad clínicamente relevante y el 54,0% síntomas leves/moderados de ansiedad; el 39,4% presentó depresión clínicamente relevante y el 52,7% síntomas leves/moderados de depresión. El puntaje total del BIPQ se asoció con síntomas clínicamente relevantes de ansiedad y/o depresión (RPa: 1,02; IC 95%: 1,01-1,02). También se observaron asociaciones con edad, modalidad laboral, aislamiento, dificultades económicas y grado de instrucción. Conclusión: La percepción de enfermedad se asoció con síntomas clínicamente relevantes de ansiedad y/o depresión en pacientes hospitalizados por COVID-19. La evaluación de la percepción de amenaza y de los síntomas emocionales puede contribuir a una atención hospitalaria más integral, incluso en el contexto clínico posterior a la emergencia internacional por COVID-19.
